The aim of the test is to detect the enzyme glutamate dehydrogenase (GDH) produced by C. difficile, which can detect infection in the absence of endotoxins.

Background: C. difficile is an anaerobic opportunistic pathogen. It is often spread by nosocomial infection. Overuse of antibiotics may play a role in its occurrence, whereby erosion of the natural gut flora provides an opportunity for the pathogenic strain to emerge. Symptoms are caused by endotoxins A and B produced by the bacteria, which can lead to colitis and diarrhoeal symptoms.
